Itachi's Susanoo has two sets of arms, which are conjoined at the elbow; the secondary arms are manifested as required.  After Sasuke gains the Eternal Mangekyō Sharingan, however, its appearance changes, the armour's mouth taking on a beak-like shape and generally becoming ghastly-looking. At this stage, it can anchor the user to the ground, preventing opponents from forcibly moving them unless they can get through Susanoo's various layers.
